A leading health organization is seeking a Lifestyle Advisor and Health Care Assistant to deliver NHS Health Checks and provide support for smoking cessation initiatives. The role involves working closely with patients to promote health improvements and conducting consultations in various settings across Greenwich. Greenwich Health deliver NHSHealth Checks, smoking cessation support, Long-Acting Reversible Contraception(LARC) services and sexual health walk in clinics. We are looking to recruit a flexible andmotivated individual who can work both independently and as part of a team in afast paced patient centred role.
The post holder will be requiredto conduct NHS Health Checks, face to face and telephone smoking cessationconsultations and provide HCA support for our Long-Acting Reversible Contraceptionclinics. The post holder will berequired to travel to various locations across the Borough of Greenwich.
The post holder will also berequired to support our busy patient call centre based at Ferryview HealthCentre, supporting patients through the service from managing the referrallist, triaging, and booking patients into clinics.
Working Pattern: 40 hoursper week (working hours between 8:00am and 8:00pm, 5 days a week Monday toSunday on a three week rota, 1 weekend day 8am to 4pm every third week)
Call Centre
Manage in-coming and out-going calls and voicemails.
Book LARC, smoking cessation and NHS HealthCheck appointments.
NHS Health Checks
Deliver the NHS Health Check sessions acrossGreenwich within the Live Well Centres.
Create a patient focused environment to helpindividuals identify how their behaviour might affect their health andwellbeing.
Providing telephone or face to face support topatients wishing to quit smoking.
Advise patients on nicotine replacement therapyoptions.
Provide on-going support to patients throughouttheir 12 week quit journey with a patient centre approach.
Long Acting Reversible Contraception Clinic
Act as a LARC assistant to our LARC clinicalstaff, ensuring the patients feel comfortable and the clinician hasappropriate, timely support.

Job Summary
The post holder will also berequired to support our busy patient call centre based at Ferryview HealthCentre, supporting patients through the service from managing the referrallist, triaging, and booking patients into clinics.
The NHS Health Check aims to helpdelay or prevent heart disease, stroke, diabetes, kidney disease and dementia.Individuals between the ages of 40 - 74, who have not already been diagnosedwith one of these conditions or do not have certain risk factors, will beinvited every 5 years to have a check toassess their risk and will be given advice to reduce or manage that risk. The NHS Health Check involves measuring bloodpressure and pulse, taking a near-patient blood sample to measure cholesterol,measuring height, weight and BMI and asking questions and providing lifestyleadvice.
Our Smoking Cessation servicesupports patients for up to 12 weeks with either face to face or over thetelephone to quit smoking. The postholder will have a patient case load to support through their smoking journey.
The post holder will supportclinicians in our LARC clinics preparing and clearing away clinical equipmentfor coil and implant procedures, supporting patients during the procedure. The post holder will provide administrationsupport the sexual health service.
Call Centre

Deal with patient queries.
NHS Health Checks
Deliver the NHS Health Check sessions acrossGreenwich within the Live Well Centres.
Carry out the NHS Health Check following protocolsand clinical guidelines.
Create a patient focused environment to helpindividuals identify how their behaviour might affect their health andwellbeing.
Communicate with individuals in line withevidence based practice.
Motivate and support individuals to makelifestyle changes via appropriate onward referrals.
Make timely referrals, following a health check,to general practice as required.
Prepare, organise and collate own paperwork,equipment and resources ready for delivery.
Process smoking referrals from practices,contacting patients within 72 hours.
Providing telephone or face to face support topatients wishing to quit smoking.
Advise patients on nicotine replacement therapyoptions.
Provide on-going support to patients throughouttheir 12 week quit journey with a patient centre approach.
Prepare, organise and collate own paperwork,equipment and resources.
Long Acting Reversible Contraception Clinic
Support LARC clinics following protocols andclinical guidelines.
Process the referrals from practices into theLARC clinics, triage and provide accurate information to ensure the patientattends the appropriate appointment and is aware of the arrangements.
Act as a LARC assistant to our LARC clinicalstaff, ensuring the patients feel comfortable and the clinician hasappropriate, timely support.
Prepare, organise and collate own paperwork,equipment and resources ready for delivery.
Training
Complete NCSCT e-learning Stop SmokingPractitioner Training
Complete both internal and external NHS HealthCheck training.
Complete practical training alongside colleaguesand clinicians for LARC clinics.
Complete all mandatory company training,including annual update / refresher training.
Attend monthly team meetings.
Take an active part in developing own knowledgeand skills, particularly the key skills and competencies required to deliverNHS Health Checks, smoking cessation and LARC support.
Keep abreast of changes within the healthindustry via the reading of literature, attending seminars and continuingprofessional development
Health &Safety
Monitor and maintain health, safety and securityof self and others.
Respond to relevant clinical /other issues orincidences to mitigate risk and maintain best practice.
Follow information governance and safeguardingprocedures.

Keep accurate and factual patient records inline with Information Governance, Data Protection Act and Greenwich Healthpolicies.
Complete timely and accurate data entry onto thecompany clinical system.

Work alongside the Live Well Development Leadand Live Well Team Leader to support service development and improvements.
Flexible working, including some evenings andweekends.
Manage and organise own time / activities toensure timely service delivery

Be a positive ambassador for Live Well at alltimes.
