Job summary

Center for Life Management (CLM) is hiring an Integrated Care Case Manager to provide care planning, resource coordination, and community-based counseling for adults and elders. The role includes telehealth and home-based activities as part of a wrap-around approach within a multidisciplinary team.

Must hold a Bachelor’s degree (or equivalent via associate degree and experience) and demonstrate strong clinical judgment, documentation, and teamwork abilities.

Qualifications

• Bachelor’s degree preferred; associate degree plus direct service experience may qualify.
• Ability to exercise sound clinical judgment in routine, urgent, and emergent situations.
• Knowledge of chronic comorbid physical health conditions and experience with related services.
• Proficient computer skills, specifically with Microsoft Office.

Responsibilities

• Provide care planning, resource identification, and service coordination.
• Map and monitor patient progress and treatment outcomes.
• Deliver counseling and community-based services in home or community settings.
• Assist with crisis intervention when needed and safe management.
