Job summary

Seven Group Holdings Limited is seeking a Leaving Care Personal Advisor in Merthyr Tydfil to support young people aged 16-25 as they transition from care to independence. The role offers flexible hours with hourly pay and the option of part-time or full-time work.

You will build trusted relationships, create Pathway Plans, and help develop life skills such as budgeting and tenancy management. You’ll advocate in multi-agency meetings and coordinate with services to promote education and wellbeing.

Qualifications

• Experience supporting vulnerable young people.
• Knowledge of relevant legislation (Social Services and Well-being Act).
• Access to a vehicle or driving licence is essential.
• Background in youth work, social care or housing support is advantageous.

Responsibilities

• Build trusting relationships with care-experienced young people and act as their primary contact.
• Create, update, and review Pathway Plans based on individual goals.
• Support development of life skills, budgeting, self-care, and tenancy management.
• Advocate for young people in multi-agency meetings to ensure their voices are heard.
• Provide tenancy support and help secure safe, suitable accommodation.
• Encourage access to education, employment, and training opportunities.
• Promote health and wellbeing by linking to appropriate services.
• Maintain accurate records using electronic systems in line with safeguarding requirements.
• Collaborate with internal and external agencies to deliver cohesive support.
