Job summary

Supply Care Solutions is recruiting an experienced Personal Advisor to join the Leaving Care Team in Shropshire. This temporary/locum role supports young people and care leavers as they transition to adulthood, managing a complex caseload across asylum-seeking and disabled cohorts.

You will undertake needs assessments, develop plans, and work with partners to promote independence and positive outcomes. DBS checks and a valid driving licence are required.

Qualifications

• Experience with care leavers or vulnerable young people.
• Experience managing a complex and diverse caseload.
• Strong knowledge of Pathway Plans, needs assessments, and transition to adulthood.
• Experience working with multi-agency professionals and partner organisations.
• Understanding of immigration, disability, or additional support needs in care leavers.
• Strong communication, assessment and report-writing skills.
• Ability to work independently within a team.

Responsibilities

• Manage a complex and diverse caseload of young people and care leavers.
• Work directly with service users to identify and achieve goals.
• Undertake needs assessments and develop plans responsive to individual needs.
• Produce and review Pathway Plans in line with guidance.
• Ensure Pathway Plans are SMART and outcome-focused.
• Collaborate with agencies and professionals in care planning.
• Produce clear, concise reports.
• Maintain professional development and supervision.
