Job summary

Lincolnshire County Council is recruiting Qualified Practitioners (Learning Disability) to support adults with learning disabilities across South Lincolnshire. You will conduct Care Act assessments, develop and review support plans, and ensure the right level of care at the right time within a strengths-based framework.

You will hold a Nursing or Social Work registration and manage a varied caseload, with potential travel across the county.

Qualifications

• Professional qualification as a Social Worker.
• Registered Nurse with HCPC/NMC registration.
• Minimum of 2 years post-qualification practice.

Responsibilities

• Provide statutory Adult Care and Health support as part of a multi-disciplinary team.
• Complete Care Act assessments and develop review care plans.
• Support individuals to achieve outcomes with person-centred plans.
• Supervise up to four non-registered staff; lead on reviews and tools.
• Travel across Lincolnshire to support adults with learning disabilities.
