Job summary

Lincolnshire County Council is recruiting Qualified Practitioners (Learning Disability) to support adults with learning disabilities across South Lincolnshire. We are seeking committed and skilled Learning Disability Nurses/Social Workers as Qualified Practitioners to join our Learning Disability South Team, based at Sleaford - Spalding.

We have two vacancies: 1 full-time 37 hours and 1 part-time 18.5 hours per week. Salary is dependent on experience.

About the Role

As a Qualified Practitioner within the Learning Disability South Team, you will play a key role in supporting adults with learning disabilities to live fulfilling, independent, and meaningful lives. You will be responsible for completing Care Act assessments, developing and reviewing support plans, and ensuring that individuals receive the right level of support at the right time.

You will hold a current Nursing or Social Work registration.

You will hold a varied caseload, including more complex cases, and will work with individuals on a long-term basis, enabling you to build strong, trusting relationships.

Main duties of the job

Working within Lincolnshire's strengths-based Conversation Model, you will:

• Complete Adult Needs Assessments and reviews under the Care Act 2014
• Develop strengths-based support plans that focus on individual outcomes
• Work alongside individuals, carers, families and partner agencies
• Promote independence, wellbeing and community connections
• Respond to safeguarding concerns and contribute to safeguarding enquiries
• Support people through periods of change, crisis and transition
• Manage a varied and rewarding caseload across your locality
• Supervise up to a maximum of 4 staff
• Leading on section 75 reviews/Decision Support Tool and CHC checklists
• Support wider Learning Disability teams with nurse rota for section 75 reviews/ Decision Support Tool and CHC checklists
• The Learning Disability focuses on working with adults who require assessments, Decision Support Tool, care planning, and ongoing interventions to meet their care and support needs. This often involves undertaking comprehensive assessments, developing person-centred support plans, and working collaboratively with families, carers, health professionals, and other partner agencies. The team covers Sleaford, Spalding, Grantham, Stamford and Bourne.

Job responsibilities

Responsible for providing statutory Adult Care and Health support as part of a multi‑disciplinary team supporting people with learning disabilities with care and support needs. You will demonstrate through practice experience a commitment to strengths based practice and supporting people to identify and build support networks to achieve their identified outcomes. You will be responsible for assessment, care and support planning, implementing and reviewing packages of care and support on a varied and more complex case load. You will be expected to work with autonomy managing more complex work and demonstrate a multi‑agency approach integrating your practice with colleagues across the health and care sector. You will provide advice, support and mentoring to other Adult Care practitioners in relation to their cases. Work may include travel outside the county.

MAIN DUTIES

• Manage an allocated caseload of clients, regularly monitor, review and evaluate changes in clients' needs and ensure that all services contribute effectively to the care plan and it remains an effective use of resources
• Undertake a range of community care and safeguarding assessments and reviews in accordance with relevant statutory requirements, identifying risks, needs and options, including self‑directed support options.
• Supervise up to four non‑registered social care practitioners e.g. Practitioner (Unqualified), including planning, allocation and checking of work including setting targets, dealing with day to day capability and conduct issues / providing basic staff welfare and support/ identify training needs
• Construct value for money care packages in collaboration with users, carers and contributing agencies in coordination with other professionals, families and individuals, and in accordance with eligibility criteria and ensuring that risk is minimised and quality of life improved/maintained.
• Work with individuals, families, carers and communities to help them make informed choices and decisions, enabling them to clarify and express their needs and contribute to service planning.
• Maintain and update case notes and other records, write reports as required, give evidence in court in relation to care or other proceedings.
• Convene and chair multidisciplinary planning and review meetings for individuals with complex needs to ensure a coordinated response to service delivery.
• Identify, track and assess young people in the transition into adulthood as specified in the transition protocol.
• Promote health in people with learning disabilities and support them to access healthcare.
• Provide advice and guidance to individuals and families on service and support providers and options including signposting to other agencies.
• Communicate with individuals and their families in ways which are appropriate to their needs
• To undertake supervision of students and participate in training and staff development programmes
• Ensure the promotion and completion of carers assessments

Qualifications

Essential

• Professional qualification as a Social Worker Or
• Registered Nurse e.g. RNLD, RGN,RNMH
• Current HCPC Social Work or NMC Nursing
• Registration
