Job summary

You will work within the Salford Community Learning Disability Service as part of a multidisciplinary team, delivering high-quality, person-centred care and promoting health and independence.

Qualifications

• Registered Learning Disability Nurse
• Band 5 with potential to Band 6 within 12 months
• Compassionate, motivated and enthusiastic with person-centred care

Responsibilities

• Promote health, wellbeing and independence for adults with learning disabilities.
• Work as part of a multidisciplinary team.
• Provide holistic, person-centred care tailored to individual needs.
• Contribute to assessments and care planning in community settings.
