Job summary

Northern Care Alliance NHS Foundation Trust is recruiting a Registered Learning Disability Nurse to join the Salford Community LD Service. The role supports adults with learning disabilities to improve health outcomes and live independently, with a clear path from Band 5 to Band 6 within 12 months.

You will work as part of a multidisciplinary team, delivering person-centred care and holistic assessments across community settings.

Qualifications

• Registered nurse with NMC registration and current practice.
• Experience in learning disability or community settings is preferred.
• Strong communication and partnership working abilities.

Responsibilities

• Promote health, wellbeing and independence for adults with learning disabilities.
• Work within a multidisciplinary team to provide person-centred care.
• Conduct holistic assessments and coordinate care with families and carers.
• Support individuals to achieve best possible health outcomes.
• Participate in induction and ongoing learning and development.
