Job summary

NHS Borders is seeking an enthusiastic Band 6 Learning Disability Liaison Nurse to join our Learning Disability Team. You will work across acute, community and primary care settings to improve the patient journey for individuals with learning disabilities and provide specialist advice, support and education to healthcare professionals, patients, families and carers.

Responsibilities

• Act as a specialist resource for learning disability care within NHS Borders.
• Support reasonable adjustments to ensure accessible healthcare services.
• Provide expert advice to multidisciplinary teams and carers.
• Promote person-centred approaches and effective communication.
• Support admissions, discharges and care planning across care settings.
• Deliver training and awareness sessions to healthcare staff.
