Job summary

Leeds and York Partnership NHS Foundation Trust is seeking a Band 6 Community Nurse to join the Assessment and Referral Team within the CLDT. The role involves triaging referrals, MDT collaboration, and delivering holistic assessments to adults with learning disabilities in community settings.

The post operates from St Marys Hospital with line management from the West and South CLDT. Flexible working and NHS pension support are available, alongside other staff development opportunities.

Qualifications

• Must be a Registered nurse RNLD preferred.
• Evidence of professional development.
• Must be a car driver with access to a vehicle.

Responsibilities

• Triage referrals into the CLDT and determine eligibility for specialist services.
• Work within MDT to coordinate across multi-disciplinary teams and agencies.
• Carry out comprehensive, person-centred holistic assessments and direct visits when needed.
