Job summary

Leicestershire Partnership NHS Trust is seeking a motivated Band 6 Practitioner to join the Learning Disability Crisis Response and Intensive Support Team (LD CRIST). You will manage your own caseload within a multidisciplinary team providing specialist support, clinical assessments and risk management to prevent inpatient admissions and placement breakdown in Leicester, Leicestershire and Rutland.

Qualifications

• Registered professional in a relevant NHS discipline
• Experience with people with learning disabilities
• Full UK driving licence and access to a vehicle

Responsibilities

• Triaging new referrals from patients, carers or professionals.
• Provide clinical assessment, planning, and oversight for community cases at risk of admission or placement breakdown.
• Offer training and advice to carers, providers and health and social care staff on meeting individuals’ needs and Positive Behaviour Support plans.
• Provide specialist advice to enable people to remain in the community and prevent inpatient admissions.
• Monitor pathways and coordinate handover to LD Community teams when longer-term monitoring is required.
