Job summary

Hertfordshire Partnership University NHS Foundation Trust is seeking a Healthcare Assistant to support service users with learning disabilities within the Enhanced Support Team. The role is based in Essex and involves working with individuals who may have complex needs.

You will receive regular supervision, keep clear records, and collaborate with the Nurse Team Leader and other agencies to achieve agreed outcomes. A focus on health promotion and patient wellbeing is essential.

Qualifications

• Evidence of academic qualifications achievement.
• NVQ Level 3 or Equivalent is required.

Responsibilities

• Provide support to an allocated group of service users as delegated by the Team Leader or registered practitioner.
• Work within the Community Specialist Learning Disability Team.
• Receive regular supervision and engage in reflective practice and training.
• Maintain accurate records of tasks and events concerning service users and report to the team.
• Promote good communication within the team and with other agencies.
• Support health promotion and healthy living for individuals as appropriate.
