The Learning Disability Acute Liaison Nurse role at York and Scarborough Teaching Hospitals NHS Foundation Trust operates within the Complex Needs Team across the York and Scarborough acute hospitals to ensure people with learning disabilities and their carers access planned, unplanned and day-treatment healthcare services.

The post holder will manage a defined caseload and coordinate complex care planning to improve patient experience.

Responsibilities

• Manage a defined caseload of patients with learning disabilities accessing acute healthcare services.
• Assess, plan, implement and evaluate specialist care and treatment programmes.
• Coordinate complex patient pathways, including admission, treatment and discharge planning.
• Provide specialist advice regarding the Mental Capacity Act, consent, best interests decisions and the Mental Capacity Act.
• Act as a specialist point of contact for patients, carers and professionals.
• Support patients and carers to understand healthcare information through accessible communication.
• Deliver learning disability awareness training and education programmes.
• Work closely with multidisciplinary teams and community services to ensure seamless care.
• Contribute to service development, policy implementation, audit and quality improvement activities.
• Maintain accurate records and comply with professional and organisational standards.
