Job summary

Trident Group in Birmingham is seeking a dedicated LD Supported Accommodation Support Worker. You will provide early, targeted support to help adults with learning disabilities build resilience, gain independence and progress through the prevention pathway.

You will assess needs, co-produce outcome-focused support plans, help customers access housing and services, and work with partner agencies to build community links while safeguarding and recording progress in line with GDPR.

Qualifications

• Experience or interest in supporting adults with learning disabilities.
• Excellent communication skills and ability to work with diverse communities.
• Ability to record support provided and produce reports; knowledge of GDPR preferred.

Responsibilities

• Assess referred customers' needs and risks against the service's eligibility criteria.
• Co-produce outcome-focused support plans reflecting customers' strengths, goals and aspirations.
• Help customers settle into and maintain their accommodation.
• Connect customers with local services and activities to build support networks.
• Lead risk assessments and safeguard in line with policies.
