Job summary

ESNEFT, East Suffolk and North Essex NHS Foundation Trust, is seeking an experienced Nurse Prescriber for the Urgent Community Response Service (UCRS). You will deliver high-quality urgent care in patients' homes and care homes with autonomy to assess, diagnose, prescribe and manage a wide range of urgent needs.

You will work within a multidisciplinary team to prevent hospital admissions and support recovery at home, developing skills in clinical leadership and service improvement while

Qualifications

• Registered nurse with current NMC registration and non-medical prescriber status.
• Experience in urgent care or community-based settings is advantageous.
• Commitment to CPD and ongoing professional development.

Responsibilities

• Deliver rapid assessments and manage urgent care in community settings.
• Independently prescribe appropriate treatments and interventions.
• Provide expert clinical advice to patients, carers and colleagues.
• Collaborate with GPs, therapists, social care and acute services.
• Support service development and clinical leadership within the team.
