Job summary

Home Group Limited is seeking a dedicated role that helps people with complex needs live more independently. You will lead support planning, coordinate Support Workers, and oversee delivery of person-centred care across daily tasks and appointments.

The role involves safeguarding, collaborating with multi-agency teams, and guiding colleagues while managing caseloads. You will contribute to empowering customers and preparing them for move-on into the community.

Qualifications

Level 3 Diploma in Care or equivalent experience (or willing to work towards it).
Experience assessing referrals and coordinating support.
Ability to work under your own initiative and remain calm under pressure.
Experience working with young people.

Responsibilities

Lead support planning and coordinate our Support Workers to deliver person-centred support
Balance support, safeguarding, admin and meetings while guiding colleagues and managing caseloads
Support Customers with daily tasks like budgeting, cooking and attending appointments
Work with multi-agency teams to safeguard and empower our customers
Help customers build independence and prepare for their move-on into the community
