Job summary

Derbyshire Healthcare NHS Foundation Trust is seeking a Lead Nurse (RMN) for the Dementia Rapid Response Team. The role leads complex cases, coordinates a seven-day service, and works within a multidisciplinary team to support older adults with dementia in or near mental health crisis.

You will supervise clinical care, manage risk, and ensure non-pharmacological interventions are central to care plans, with focus on home and community-based support.

Qualifications

• Recent post-qualification nursing experience as an RMN.
• Knowledge of the MHA, Mental Capacity Act and legal frameworks.
• Mentorship/SSSA training or willingness to complete.
• Able to meet travel requirements for the post.

Responsibilities

• Lead complex cases and make complex clinical decisions within a multidisciplinary team.
• Provide flexible community support to people with dementia.
• Undertake the duty role: triage referrals, allocate visits and deliver handover.
• Complete initial assessments, assess risk and formulate safety plans.
• Formulate person-centred care plans emphasizing non-pharmacological interventions.
• Apply the Mental Capacity Act and Mental Health Act when required.
• Provide in-reach to inpatient wards and attend 117 meetings.
• Visit patients at home and in care settings across the catchment area.
• Liaise with professionals and agencies as part of the MDT.
• Travel independently and work shifts across a seven-day service.
