University Hospitals of Derby and Burton NHS Foundation Trust is seeking a Lead Nurse for the Dementia Rapid Response Team. The role focuses on early discharge from acute settings and preventing hospital admissions by delivering short-term, intensive interventions to older adults with dementia.
The Lead Nurse will make complex decisions, lead shifts, cover duties, and develop detailed assessments and person-centred care plans. Training and appraisal support will be provided.
