Job summary

Cornerstone is seeking an experienced Lead Practitioner to head a team delivering person-centred health and social care support for young people in North Lanarkshire. The role is part-time (25 hours) on a fixed-term basis for 12 months to provide maternity cover.

You will mentor Support Practitioners, guide care planning and ensure high-quality delivery while promoting independence and practical life skills for young people.

Qualifications

• Experience of managing and supporting people.
• Experience within the Health & Social Care sector.
• A passion for delivering high-quality, person-centred support.
• An understanding of supporting young people to develop independence and life skills.
• Knowledge of SSSC Codes of Practice and Health & Social Care Standards.
• Excellent communication and interpersonal skills.
• The ability to lead, motivate and develop a team.
• A strong understanding of professional boundaries and accountability.
• A commitment to achieving an SVQ Social Services & Healthcare SCQF Level 7 and PDA in Health & Social Care Supervision, if not already attained.

Responsibilities

• Lead, support and inspire a team of Support Practitioners to deliver high-quality, person-centred support.
• Provide guidance and mentorship to your team to help young people develop confidence, independence and life skills.
• Collaborate with the Service Lead to ensure the service operates effectively and within requirements.
• Must hold a full UK manual driving licence.
