Job summary

Redwood Prairie Rehabilitation & Nursing is seeking a Licensed Social Worker to provide psychosocial assessments, care planning, and counseling to residents and families. Qualifications

• Licensed Social Worker in good standing.

Responsibilities

• Conduct psychosocial assessments and care planning for residents.
• Advocate for residents' rights and deliver person-centered care.
• Provide counseling to residents and families in adjustment, crisis, and discharge planning.
• Lead the social services department and collaborate with the interdisciplinary team.
• Prepare for surveys, audits, and QAPI initiatives.
• Coordinate community resources and educate families on facility processes.
• Identify trends and contribute to performance improvement initiatives.
