Job summary

ICL is seeking a licensed social worker (LMSW/LCSW) to join our team in the United States. The role involves psychosocial assessments, therapy, and care coordination across services for individuals and families.

The position requires NY licensure, authorization to work in the U.S., and the ability to coordinate treatment plans with a multidisciplinary team. Experience with the target population is preferred and driving may be required.

Qualifications

• Licensed by New York State as an LMSW or LCSW.
• Eligible for full and unconditional Medicaid/Medicare participation.
• Authorized to work in the U.S.
• Experience with the population served by the clinic preferred.
• May require a valid NYS driver's license depending on assignment.

Responsibilities

• Interview individuals and complete psychosocial assessments.
• Obtain psychosocial histories from individuals and families.
• Assess treatment needs and determine levels of care.
• Develop, review, and update individualized treatment plans.
• Provide individual, group, and family therapy.
• Coordinate services with community providers and ensure discharge planning.
• Document progress notes, treatment recommendations, and discharge plans.
• Collaborate with the interdisciplinary team to ensure quality care.
