Job description

We are seeking experienced and compassionate Social Workers to provide high-quality, person-centred support to individuals with a range of complex needs. The successful candidates will be responsible for assessing needs, developing and reviewing care and support plans, managing complex cases, and working collaboratively with service users, families, healthcare professionals and other relevant agencies.

Key responsibilities may include conducting Care Act assessments, completing statutory reviews, supporting the development and management of complex care packages, and facilitating hospital discharge and Discharge to Assess (D2A) pathways. Candidates may also work with individuals requiring support within mental health, learning disabilities and older people's services.

The role requires strong assessment, communication, case management and report-writing skills, alongside the ability to manage a caseload effectively while promoting independence, safeguarding and positive outcomes for service users.
