Cornwall Partnership NHS Foundation Trust in Camborne is seeking a dynamic, motivated Community Nursing Team Leader to guide the Camborne Community Nursing Team. The role focuses on holistic nursing assessment, safe evidence-based care, and enabling patients to remain at home.

You will manage a clinical caseload with guidance from team leads, develop neighbourhood team working, and lead staff to deliver high-quality care, support timely discharge, and promote patient independence.

Formación

• Current professional registration with the NMC is required.
• Experience of working with patients with complex and challenging needs.
• Postgraduate specialist training to degree level in a relevant subject area.

Responsabilidades

• Assess and treat patients with complex health and social care needs.
• Manage a clinical caseload within a community nursing team with guidance from team leads.
• Provide holistic nursing assessments and high-quality, safe, evidence-based care to enable patients to remain at home.
• Develop neighbourhood team working and support safe hospital admission avoidance and timely discharge.
• Provide clinical leadership to staff within the team.
