Job summary

Cornwall Partnership Foundation NHS Trust is recruiting a Senior Practice Nurse to lead the general practice nursing team and ensure safe, evidence-based care across the patient population.

The post holder will manage nursing resources, work with GPs and the Practice Manager to deliver priorities, conduct nurse appraisals, support vaccination programs, and participate in chronic disease management within NICE guidelines.

Qualifications

• Registered nurse with current NMC registration.
• Experience leading a nursing team in a primary care setting.
• Knowledge of immunisation, screening and chronic disease management.

Responsibilities

• Lead and develop the practice nursing team to deliver care.
• Manage nursing resources, rotas, appraisals and revalidation.
• Coordinate vaccination programmes and chronic disease management.
