Job summary

Kent Community Health NHS Foundation Trust is seeking a Band 6 Community Nurse Team Leader to join our Maidstone team. You will lead a caring, progressive unit delivering home-based and community care in collaboration with GPs, mental health professionals, social workers, and voluntary services.

You will support evidence-based care planning, mentor others, and promote flexible working patterns within an inclusive NHS environment.

Qualifications

• Registered health care professional with the Nursing and Midwifery Council.
• Experience of managing complex caseloads and mentoring others.
• Ability to use IT systems and databases.

Responsibilities

• Lead the Community Nursing Team to deliver care at home and in other settings.
• Support evidence-based care planning and ensure a positive patient experience.
• Encourage and support transition into Community Nursing and develop career pathways.
