Job summary

Locala in West Yorkshire is recruiting for a Band 6 Senior Community Nurse. This full-time role (37.5 hours) leads and coordinates community care across North Kirklees and Greater Huddersfield, providing operational leadership in the absence of the Team Leader.

You will combine clinical expertise with leadership, mentor Band 5 nurses and drive service improvements, while promoting patient-centred care and strong teamwork within a supportive environment.

Qualifications

• Registered Nurse with substantial community nursing experience.
• Proven leadership and mentoring skills.
• Excellent communication and problem-solving abilities.
• Ability to delegate and coordinate.

Responsibilities

• Lead and coordinate care for patients with complex needs.
• Provide guidance, supervision and mentorship to Band 5 nurses.
• Delegate tasks to ensure efficient resource use.
• Support service development and quality improvement projects.
• Act as escalation point for clinical decisions and safeguarding.
• Promote self-care and independence for patients with chronic conditions.
• Collaborate with multidisciplinary teams to prevent admissions.
• Provide leadership in the absence of the Team Leader.
• Foster a positive, learning-focused team culture.
