Job summary

HCRG Care Group in Sittingbourne is seeking a Band 6 Team Sister/Charge Nurse to lead our Community Nursing team. The role combines hands-on clinical care with leadership responsibilities, managing a defined caseload and coordinating care delivery in patients' homes.

You will provide clinical leadership, support decision-making, and work closely with GPs, social care and specialist services to ensure safe, patient-centred care.

Qualifications

• Current NMC registration
• Teaching and Assessing qualification or equivalent
• Evidence of ongoing professional development
• Minimum 3 years' experience as a registered nurse
• Experience in community and acute or hospital settings
• Experience supervising colleagues
• Strong communication and IT skills
• Ability to work autonomously and make sound clinical decisions

Responsibilities

• Manage a defined caseload of patients with complex nursing and health needs within the community
• Assess, plan, implement and evaluate individualised care plans to support positive patient outcomes
• Support patients with long-term conditions, palliative and end‑of‑life care, wound management and other community nursing interventions
• Participate in the triage of referrals and support effective allocation of visits according to clinical need
• Provide clinical leadership and act as a role model for high standards of patient care
• Lead, supervise and support junior colleagues and students to develop their knowledge, skills and confidence
• Participate in staff development, supervision, appraisals and competency assessment
