Job summary

Praxis Care seeks a Team Leader to coordinate and deliver person-centred support for adults with health and social care needs. You will design individual and group programmes, supervise Support Workers, and ensure safety, dignity and independence in line with policies.

The role requires 2 years in health and social care, a willingness to pursue a professional qualification, a full UK driving licence, and rights to work in the UK. Salary is based on a 39-hour week and pro-rated as needed.

Qualifications

• 2 years’ experience in a health and social care setting or equivalent
• Willingness to pursue a professional qualification from the listed options
• Right to Work in the UK and registration requirements as applicable

Responsibilities

• Plan, coordinate and implement safe transitions into the service and community
• Liaise with families and multidisciplinary teams to advocate for needs
• Develop and implement individual and group support programmes
• Oversee assessments, reviews and records in line with policy
• Lead, supervise and manage Support Workers, including attendance and disciplinary procedures
