Real Life Options Group is seeking a Care Coordinator in Falkirk and surrounding areas to lead a homecare team delivering person-centred support. You will manage day-to-day operations, rosters, risk assessments and audits while developing staff through supervision and appraisals.

You will liaise with social workers, families and external agencies and deputise for the Service Manager when required. This permanent role offers a rewarding opportunity to make a difference within our care-at-home

Qualificações

• SVQ Level 3 in Health & Social Care (or equivalent).
• Minimum 2 years’ experience in a similar social care role.
• Proven supervisory and leadership experience.
• Excellent communication and interpersonal skills.
• Driver with access to a vehicle.

Responsabilidades

• Lead and support staff to deliver person-centred home care.
• Manage rotas, risk assessments, audits and health & safety compliance.
• Supervise and develop Support Workers through appraisals.
• Embed the Person-Centred Support model across the service.
• Liaise with social workers, families and external agencies.
• Deputise for the Service Manager when required.
