A exciting opportunity has arisen for a Band 6 Learning Disability Primary Care Liaison Nurse within the Aneurin Bevan University Health Board. The role focuses on improving physical health and healthcare experiences for people with learning disabilities, advancing access to primary care and mainstream services.

Qualifications

• NMC registered nurse with registration in the UK.
• Experience working with people with a learning disability is essential.
• Ability to communicate effectively with patients and colleagues, including Welsh speakers.

Responsibilities

• Provide specialist learning disability health advice and guidance.
• Collaborate with Primary Care, LD services, families and carers.
• Promote Annual Health Checks and reduce health inequalities.
• Identify barriers to healthcare and promote reasonable adjustments.
• Deliver education and training for professionals, people with a learning disability, families and carers.
• Contribute to audit, data collection and service development.
• Manage autonomous workload in a multi‑agency setting.
