Job summary

The Learning Disability Primary Care Liaison Nurse role at Aneurin Bevan University Health Board focuses on improving physical health and healthcare access for people with a learning disability across the Gwent region.

The post holder collaborates with individuals, families, carers, Primary Care and Public Health Wales to promote Annual Health Checks, reasonable adjustments, and better health outcomes. Travel within the health board area is required.

Qualifications

  • NMC registered nursing qualification and license.
  • Experience with learning disability health needs and services.
  • Ability to work autonomously and collaboratively with multiple teams.

Responsibilities

  • Provide specialist advice and support regarding health needs of people with a learning disability.
  • Work with Primary Care, LD services, individuals, families, carers and partner organisations.
  • Promote Annual Health Checks and reduce health inequalities.
  • Support Primary Care to identify people with a learning disability and maintain registers.
  • Identify barriers to healthcare and promote reasonable adjustments and accessible information.
  • Assist people with a learning disability to access mainstream healthcare services.
  • Promote best practice in capacity and consent.
  • Plan and deliver education and training to professionals and carers.
  • Contribute to audit, evaluation, data collection and service development.
  • Manage autonomous workload within a multi-agency setting.
  • Champion rights, health and wellbeing of people with a learning disability.
  • Welsh and/or English speakers are welcome to apply.
