Job summary

Powys Teaching Health Board is seeking a Community Nurse (Learning Disabilities) to deliver high-quality, person-centred care within a multi-disciplinary team across our learning disability services in Powys. You will assess needs, plan and evaluate nursing care while supporting smooth transitions from Children's into Adult services.

We welcome Welsh speakers but training is provided for language development.

Qualifications

• Registered Nurse (Learning Disability) with current Nursing and Midwifery Council registration
• Postgraduate level in specialist area
• Evidence of ongoing professional development relevant to nursing practice
• Understanding of the health needs of people with learning disabilities, including health inequalities
• Knowledge of person-centred care and therapeutic relationships
• Awareness of safeguarding responsibilities for children and adults
• Understanding of risk assessment and clinical risk management in community settings
• Knowledge of legislation including Mental Capacity Act, Equality Act, and Human Rights-based approaches
• Experience of multidisciplinary and multi-agency working in community services
• Knowledge of accurate record-keeping and digital documentation requirements
• Practice Supervisor and Practice Assessor qualification (or willingness to undertake)
• Training in learning disability nursing (e.g., Positive Behavioural Support)
• Training in behaviour support, mental health awareness, or communication for complex needs
• Awareness of trauma-informed practice and public health initiatives for learning disabilities

Responsibilities

• Carry out holistic nursing assessments and deliver person-centred interventions
• Build and maintain therapeutic relationships with people with learning disabilities
• Prioritise and organise a caseload in a community setting
• Identify and respond to clinical risk
• Work autonomously and in a multidisciplinary team
• Adapt communication to diverse needs
• Engage in supervision and CPD
• Exercise sound nursing judgement
• Develop and monitor care plans
• Involve referrers and service users in feedback
• Welsh language skills or willingness to learn
• Contribute to quality improvement projects
• Support health promotion for learning disabilities
