Job summary
The Sidmouth Hospice at Home Community Nursing Team deliver 24hour specialist end of life care to patients and their families during their last year of life. The successful candidate will be welcomed into our caring team, working alongside our sister charity, Sidmouth Hospice at Home, and based in our new building at Sidford. As a specialist palliative care nurse you will have on-going access and be positively encouraged to engage in your own learning and professional development, at your own pace and with supportive clinical supervision.

The Hospice at Home service is managed by the Royal Devon and funded by the Sidmouth Hospice at Home charity. The Sid Valley Integrated Model for Palliative and End of Life Care integrates the available services within the community into a one-team approach to provide the best possible care for all patients who are diagnosed by their GP as palliative, regardless of diagnosis or complexity, guided by the individual needs and wishes of the patient and their carer.
The service provides specialist palliative care 7 days a week, 365 days a year, with shifts covering 08.00-16.00, 10.00-18.00 and 12.00-20.00. After 20.00 hours an RGN is on-call to respond to our patients who need advice or a visit overnight, thus providing specialist end of life care and carer support 24/7 and avoiding unnecessary and unwanted hospital admissions.
The service is part of community care provision in Sidmouth for patients registered with a Sidmouth GP. You will need to have cared for palliative care patients. Community experience is desirable but not essential. You will have opportunity to learn and gain experience through clinical reflective practice and continued professional development.
You will be joining a professional, specialist palliative care team, who work alongside our health and social care colleagues both in the community and the acute care sectors.
Working Hours: 22.5 hours per week

Pay: The salary advertised for this position reflects the Agenda for Change 2025/6 rates, inclusive of a 3.6% pay award which will be implemented later this year. If you join before the implementation date, the pay award will be backdated to your start date.
