Job description

The post holder will exercise a high degree of personal and professional autonomy, with the ability to plan and reach complex and critical judgments, demonstrating decision-making skills. The role involves delivering high-quality clinical care within the Integrated Healthcare Service for the prison, including acute & primary care, long-term conditions management, enhanced assessment and diagnostic services, and health promotion activities, aligned with the NHS Plan, public health indicators, and National Service Frameworks.

The post holder will provide robust expert clinical skills with a sound understanding of evidence-based nursing practice, ensuring quality and outcome-driven practice daily. They will support the development of evidence-based practice in their specialist field and promote research as appropriate. Participation in managerial and professional supervision, training, and annual appraisal is required, integrating these into team practice.

Responsibilities include implementing effective patient-centered care planning tools, long-term condition registers, and ensuring consistent delivery across all prisons within the service. The post holder will collaborate with the Clinical Lead, GPs, and other clinicians to meet service needs, working within skilled multidisciplinary teams comprising healthcare staff and strategic partners, managing a single referral pathway.

Training will be undertaken to maximize screening, health promotion, prevention activities, including vaccinations. A named care coordinator will be allocated for patients with complex needs to ensure proactive, evidence-based interventions, health promotion, and management of acute and long-term conditions, using a stepped care approach. The post holder will develop and regularly review individualized care plans and risk assessments, ensuring MDT meetings are tailored to the establishment's needs.

The post holder will undertake training in evidence-based CBT approaches to enhance care quality and support patients in managing their health across various aspects, from oral health to long-term conditions. Health promotion will be embedded throughout the service.

The post holder will liaise with senior Trust personnel to share expertise, promote ideas, and ensure collaborative working across Directorates. Working effectively in challenging environments, they will be aware of environmental hazards and ensure staff awareness of health & safety policies in accordance with the Health & Safety at Work Act.

Compliance with Trust policies and guidelines is essential. Familiarity with Prison Standards, Standing Orders, Circular instructions, Notices to Staff, and other relevant materials is required to function effectively within a prison setting.
