Job summary

Impact Life is seeking an experienced Practice Coordinator to lead the Kent County Social Prescribing & Integrated Outpatient Services program in Dover. You will oversee operations, develop workflows, and coordinate across medical, behavioral health, peer recovery, and community services to improve engagement and access to recovery supports.

The role requires a Bachelor’s degree and at least three years in behavioral health or related fields, with strong leadership, collaboration, and data

Qualifications

• Bachelor's degree in social work, behavioral health, public health, human services, healthcare administration, or related field. Equivalent education and experience may be considered.
• Minimum of three years of relevant experience in behavioral health, SUD services, recovery services, or related field.
• Experience coordinating programs, services, or multidisciplinary care.
• Experience supervising, coaching, or providing day-to-day leadership to staff.
• Strong understanding of substance use disorder and recovery-oriented care.
• Ability to travel locally throughout Kent County and to partner locations.

Responsibilities

• Oversee day-to-day operations of the Social Prescribing & Integrated Outpatient Services program.
• Coordinate workflows to support low-barrier access and engagement.
• Link medical, behavioral health, peer recovery, and community services.
• Facilitate connections to treatment, housing, transportation, and recovery supports.
• Collaborate with Nurse Practitioner, Peer Recovery Specialist, and partners.
• Provide supervision and day-to-day leadership to staff.
• Participate in recruitment, onboarding, training, and performance management.
• Build partnerships with healthcare, behavioral health, SUD treatment, and community orgs.
• Strengthen referral pathways and community awareness.
• Monitor engagement, referrals, and program performance measures.
• Coordinate data collection and reporting for leadership and funders.
• Ensure timely, quality program documentation and compliance.
• Lead quality improvement initiatives and ensure alignment with grants.
