Impact Life in Dover, Delaware, invites a Family Nurse Practitioner (FNP) / PMHNP to join its Rural Health Hub and Integrated Outpatient Services. You will provide primary and preventive care, behavioral health assessment and treatment, MOUD, and coordinate care with a multidisciplinary team to expand access for individuals facing barriers to traditional healthcare.

Qualifikationen

• Current, unrestricted Nurse Practitioner license in Delaware.
• Dual certification as FNP and PMHNP.
• Board certification through ANCC/AANP or equivalent.
• Active DEA registration and prescriptive authority.
• Ability to prescribe MOUD per federal/state requirements.
• Minimum 2 years in primary care/behavioral health/integrated care.
• Understanding of integrated care models and health equity.

Aufgaben

• Conduct comprehensive medical and behavioral health assessments.
• Diagnose and treat acute and chronic conditions within scope of practice.
• Provide primary and preventive healthcare, including screenings and referrals.
• Assess, prescribe, and manage MOUD.
• Prescribe and manage psychotropic medications for chronic conditions.
• Develop individualized, person-centered treatment plans.
• Provide health education on preventive care and recovery.
• Incorporate social prescribing by linking to community resources.
• Coordinate care across medical, behavioral health, SUD treatment and community partners.
• Maintain accurate EHR documentation.
• Participate in quality improvement and program evaluation.
