Impact Life is hiring a Certified Community Health Worker for our Rural Health Hub in Dover, Delaware. You will bridge participants with healthcare systems and community resources, supporting Kent County and surrounding rural areas.

Responsibilities

  • Conduct community-based outreach to engage individuals facing barriers to care.

  • Assist with scheduling, referrals, and follow-up for appointments.
  • Coordinate connections to primary care, preventive, behavioral health, and recovery services.
  • Facilitate warm handoffs and track referrals and outcomes.
  • Identify social and practical barriers affecting healthcare access.

About the Role

Impact Life is seeking a Certified Community Health Worker to join our Rural Health Hub team in Dover, Delaware.

The Certified Community Health Worker serves as a primary bridge between participants, healthcare systems, and community resources. This role supports individuals and families throughout Kent County and surrounding rural communities who may experience barriers to traditional healthcare and helps improve access, engagement, and continuity of care.

The Community Health Worker conducts outreach, provides health education, assists with healthcare appointments and referrals, addresses social determinants of health, conducts follow-up, and helps participants overcome barriers that may prevent them from accessing or remaining connected to care.

Key Responsibilities
  • Conduct community-based outreach and engage individuals who may experience barriers to traditional healthcare.
  • Serve as a trusted bridge between participants, healthcare providers, and community resources.
  • Provide health education and help participants understand and navigate available healthcare and community services.
  • Assist with scheduling and navigating healthcare appointments, referrals, and follow-up services.
  • Coordinate connections to primary care, preventive health, specialty care, behavioral health, SUD treatment, recovery support, and community resources.
  • Facilitate warm handoffs and follow up on referrals to strengthen successful connections to services.
  • Identify social and practical barriers affecting healthcare access.
  • Help participants navigate transportation, housing, food, financial assistance, healthcare access, social services, and other resources.
  • Support access to telehealth, health education, and community-based services.
  • Collaborate with the RN Program Manager, Nurse Practitioner, Medical Assistant, recovery support staff, behavioral health providers, and community partners.
  • Maintain timely and confidential documentation of outreach, referrals, follow-up, and services.
  • Track referrals, successful linkages, and required program outcomes.
  • Participate in interdisciplinary care coordination, program implementation, and quality improvement activities.
Required Qualifications
  • Current Community Health Worker certification required.
  • High school diploma or equivalent.
  • Knowledge of or ability to learn healthcare, behavioral health, substance use disorder treatment, recovery, and community resource systems.
  • Ability to effectively support individuals and families experiencing barriers to healthcare and other social determinants of health.
  • Strong interpersonal, communication, organizational, and documentation skills.
  • Ability to maintain professional boundaries and protect confidential participant information.
  • Ability to work independently in community settings while collaborating with a multidisciplinary team.
  • Ability to travel throughout Kent County and surrounding rural communities as required.
Preferred Experience
  • Experience in community health, human services, healthcare navigation, care coordination, outreach, recovery support, or a related setting.
  • Experience supporting individuals with complex healthcare, behavioral health, substance use, housing, transportation, food, financial, or other social needs.
  • Knowledge of Delaware healthcare, behavioral health, SUD treatment, recovery, and community resource systems.
  • Experience with rural health, low-barrier services, health equity initiatives, or grant-funded community health programs.
