Job summary

Barnsley Hospital is seeking a band 6 Specialist Occupational Therapist to join the Transition Unit, supporting patients through rehabilitation and discharge to their home environment. The role involves clinical leadership within a multidisciplinary team, duties including assessments, equipment prescription, discharge planning and documentation.

A strong focus on service improvement and NHS guidelines is expected.

Qualifications

• Broad range of clinical experience including NHS acute hospital settings.
• Experience of clinical audit and research.
• Diploma or Degree in Occupational Therapy; HCPC registration; CPD portfolio.

Responsibilities

• Provide specialist advice to relatives, carers and MDT to promote OT roles.
• Advise on safe use of equipment, adaptations, and treatment techniques.
• Prescribe equipment and manage access, ensuring safe use per protocols.
• Document changes in patient condition and progress to MDT members.
• Produce discharge summaries, risk assessments and recommendations for individual needs.
• Represent OT in MDT meetings to coordinate multidisciplinary care.
• Oversee caseloads of junior staff within designated areas.
