You will complete Care Act Assessments, hold a caseload, and provide advice and signposting to residents. The LAP team supports residents with multidisciplinary care to look at needs holistically, helping prevent longer-term challenges and hospital admissions.

The Social Care Practitioner role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers and Occupational Therapists.
The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.
