Job summary

The Royal Borough of Windsor and Maidenhead is recruiting a Social Care Practitioner to join the Locality Access Team (LAP). This full-time, permanent role offers hybrid working and a salary of £26,895 – £30,132 per annum.

You will complete Care Act Assessments, hold a caseload, and provide advice and signposting to residents. The LAP team supports residents with multidisciplinary care to look at needs holistically, helping prevent longer-term challenges and hospital admissions.

Qualifications

  • An individual with a background or experience in working with people in a care setting/or similar.
  • A good understanding of IT with skills including MS Office, Word, and Excel.
  • Must hold a UK driving licence.

The Role

The Social Care Practitioner role is within the Integrated care team (LAP) which consists of Community Matrons, Mental Health Practitioners, Social Workers and Occupational Therapists.

The LAP team involves supporting people with multidisciplinary support to look at the needs of the resident in a holistic way helping to prevent longer term challenges and admissions to hospital.

Your Role Will Involve
  • Completing Care Act Assessments with residents and their carers.
  • Holding and managing a case load.
  • Assisting the duty function of the team, and providing advice and signposting to residents.
