Job summary

Royal Borough of Windsor and Maidenhead (RBWM) is offering a full-time Social Care Practitioner role in Maidenhead. The post is hybrid with a salary of £26,895–£30,132 per annum and 37 hours per week.

The LAP team supports residents through multidisciplinary care, with priorities on Care Act assessments, caseload management, and providing guidance to residents and carers. A UK driving licence and IT proficiency are required.

Qualifications

• Experience in care settings or related roles.
• Good IT skills, including MS Office (Word, Excel).
• Must hold a UK driving licence.

Responsibilities

• Complete Care Act Assessments with residents and carers.
• Hold and manage a caseload.
• Assist the duty function of the LAP team and provide advice and signposting.
