Job summary

NELFT NHS Foundation Trust is seeking an Integrated Care Assistant to join our inpatient rehabilitation team. The role focuses on supporting patients through rehabilitation, promoting independence, and assisting with therapeutic activities under the guidance of registered professionals.

You will work closely with a multidisciplinary team to deliver high-quality, person-centred care and help patients achieve their goals as they prepare to return home or to a community setting.

Responsibilities

  • Support patients with personal care needs while promoting independence and dignity.
  • Assist patients to participate in rehabilitation programmes and therapeutic activities.
  • Encourage and motivate patients to achieve their individual rehabilitation goals.
  • Support mobility, transfers, and exercise programmes as directed by therapists and nursing staff.
  • Monitor and record patients' progress, reporting any concerns to the multidisciplinary team.
  • Assist with assessment activities and the implementation of treatment plans under supervision.
  • Support patients with activities of daily living such as washing, dressing, meal preparation, and community skills.
  • Contribute to maintaining a safe, clean, and therapeutic environment.
  • Work collaboratively with Occupational Therapists, Physiotherapists, Nurses, and other healthcare professionals to deliver high-quality patient care.
  • Complete documentation accurately and in a timely manner.
  • Participate in team meetings, supervision, and training opportunities.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.

Probationary Period

This post will be subject to a probationary period.

High Cost Area Supplement Outer London (B&D, Redbridge, Havering and Waltham Forest, CEME)

This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
