Job summary

North East London NHS Foundation Trust (NELFT) invites applications for an Assistant Practitioner / Nurse Associate to support person-centred care for adults in community settings. You will work with caseload managers and a multidisciplinary team to enhance health, independence, and recovery while supporting long-term condition management and rehabilitation.

• Contribute to admission avoidance strategies and facilitate timely hospital discharges.
• Provide compassionate care that upholds patient dignity, choice, and respect.

The Role

As a Assistant Practitioner / Nurse Associate, you will play a key role in supporting the delivery of person-centred care to adults in community settings. Essential Skills and Qualifications

• Proven ability to deliver direct patient care, including clinical observations, wound care, and delegated nursing interventions.
• Experience supporting the management of long-term conditions and rehabilitation programmes.
• Demonstrated ability to promote patient independence, wellbeing, and self-management.
• Competence in monitoring and reporting changes in patients’ conditions.
• Proficiency in maintaining accurate and detailed patient records using electronic systems.
• Strong collaborative skills, with experience working in multidisciplinary teams.
• Understanding of strategies to support admission avoidance and timely hospital discharge.
• Commitment to providing person-centred, compassionate care that respects patient dignity and choice.

Starting with NELFT

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Essex.

Probationary Period

This post will be subject to a probationary period. Internal applicants are exempt from the probationary period (unless you are an internal applicant currently part way through a probationary period or currently a bank member of staff).

High Cost Area Supplement – Outer London

(B&D, Redbridge, Havering and Waltham Forest, CEME) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,714 to a maximum of £5,941 p.a. pro rata for part time).
