Job summary

Enaahtig Healing Lodge & Learning Centre in Ontario is seeking an Intake Coordinator to manage client intakes, assessments and referrals for the North Lodge residential programs. You will be the first point of contact and work with Indigenous communities to connect people with appropriate services.

The role requires a post-secondary credential, strong interview and assessment skills, and the ability to balance independent work with team collaboration.

Qualifications

• Post-secondary diploma or bachelor’s degree from a recognized university.
• Experience working with Indigenous communities.
• Interview and assessment skills.
• CPR/First Aid training or willingness to obtain.

Responsibilities

• First point of contact for intake, assessment and referrals to North Lodge programs.
• Maintain waitlists and communicate regularly with clients.
• Prepare and present written intakes/assessments to the treatment team.
• Develop linkages to facilitate client access to services.
• Ensure clients understand program policies and processes.
• Participate in outreach and staff meetings.
• Coordinate Intake Review meetings with staff.
• Assist in designing culturally appropriate intake and case management approaches.
• Provide support for communication, program statistics and data uploads.
• Liaise with agencies and maintain a network with area service providers.
• Provide referrals ensuring continuity of care.
• Other duties as required.

Position: Intake Coordinator

Term: Full Time

Supervisor: North Lodge Program Manager

Location: Alban, ON

Closing Date: Open until filled

Scope:

This position is under the direct supervision of the North Lodge Program Manager, and overall supervision of the Executive Director. The Intake Coordinator will be responsible for the intakes and assessments for the clients to the North Lodge residential programs.

Responsibilities:

· First point of contact to community requests for intake, assessment and referrals to the North Lodge residential programs;

· Maintain waitlists for programs and ensure communication regularly with clients on waitlist;

· Ensuring written intakes/assessment reports are completed and presented to treatment team in a timely manner;

· Ensure communication avenues and linkages are developed to facilitate client access to appropriate services;

· Ensure clients are familiar with all policies, protocols and processes of the programs;

· To participate in internal and external outreach;

· To attend and participate in staff and team meetings as required;

· To coordinate all Intake Review meetings with the appropriate staff;

· Assist in designing and developing culturally appropriate approaches to intake, assessment and case management processes;

· To provide support to the teams in the management of communication, information, program statistics and data uploads;

· Liaise with other agencies, maintaining a network with area service providers;

· To provide appropriate referrals ensuring a continuum of care; and

· Other duties as required.

Qualifications:

· To possess a post-secondary diploma or bachelor’s degree from a recognized university;

· Experience working with Indigenous communities;

· Interview and assessment skills;

· Have or are willing to obtain CPR/First Aid, NVCI and other training as required;

· Be able to work independently and team oriented;

· Must provide a clear Vulnerable Sector’s Check;

· Must have a valid driver’s license and reliable transportation; and

· Be a role model in the community. Salary & Benefits:

· Base Salary: $55,000 to $60,320 per year

· Additional 4% vacation pay on top of Base Salary

· 3-Weeks Paid Vacation Time and Sick Time (accrued)
