Job summary

Lancashire and South Cumbria NHS Foundation Trust is seeking a Forensic Discharge Pathway Facilitator (Inpatient Social Worker) to join Guild Lodge. The role requires a qualified Social Worker with Social Work England registration and a focus on secure mental health and offender health.

The post holder will work closely with a MDT, lead holistic assessments, and coordinate discharge planning to prevent delays, including liaising with authorities for capacity and funding decisions.

Qualifications

• Registered Social Worker with current UK registration.
• Degree level qualification or equivalent level of skills, knowledge and experience.
• Evidence of continuing professional development.

Responsibilities

• Lead social care input and discharge planning for Guild Lodge patients.
• Complete holistic social work assessments pre-admission.
• Identify social care needs and coordinate with Local Authorities/Decision Maker.
• Attend MDT meetings and care planning meetings.
• Support with funding applications and discharge to the community.
