• Provide senior clinical leadership and team management to the Inpatient Social Care Team.
• Drive service improvement and operational delivery.
• Champion evidence-based practice and specialist knowledge in social care for forensic settings.
• Support hospital discharge planning and signposting, working with partners and agencies.
• Lead liaison with social care partners, commissioners and external organisations.
