Job description

To support and work in partnership with the mental health social worker and wider multi-disciplinary team. This will be to provide a range of support to an agreed number of service users to ensure that the social perspective of mental health is promoted and to work within a strength-based perspective. To support with assessing, planning, implementing, and reviewing funded and non-funded packages of care and support. The role will also require working with people directly to support them to develop independence and to work with them to achieve better outcomes to improve quality of life.


To provide support and give time to an allocated group of service users in order to promote their recovery and maintain them in a community environment helping them to build their own support network. To provide regular and practical support to service users in developing and managing independence and maintaining dignity and self-respect. To support service users to identify their needs and to assist the mental health social worker to assess, plan, implement and evaluate individual care and support plans. To have the individual service users needs at the fore at all times and use agreed values and skills to underpin their day to day work.


To work as part of a team under direction of the mental health social worker to focus on the direct needs of service users, working across boundaries of care, organisation and role, co-ordinated through the Care Programme Approach, Care Act 2014 and s.117 aftercare Mental Health Act. To build rapport with service users this will include working with them and developing a working relationship. To help ensure the service user understands and has a clear pathway of care across sector/agency boundaries with key contact points/named individuals. To support service users to engage effectively with the agreed CPA and care and support plans and access appropriate services.


To positively promote independent living of service users within the community by maintaining extensive knowledge and links with community resources and actively supporting service users to use them e.g. accessing clubs, centres, libraries and daytime services. Where appropriate liaise with other agencies e.g. Benefits Agency, Housing Department, gas and electricity services.


To provide support with daily living including self-care and home management e.g. budgeting, cooking, cleaning and shopping in line with the person agreed eligible needs. To help service users to access information about resources including, for example, health services (physical health) benefits, welfare rights and health promotion topics. To contribute to the delivery of CPA planning by providing regular and practical support to service users and their carers.


To encourage service users to act in response to any changes including relapse. Or, if needed, in the interests of the service users health or safety or the protection of others, for the mental health social care support worker to initiate appropriate action, within organisational policies and procedures.
