We have an exciting opportunity for a Band 6 Social Worker to join us in delivering high quality community mental health care within Stevenage and North Hertfordshire.

Requirements

• Experience working in a community-based setting.

Role Responsibilities

1. Deliver care to an allocated caseload of clients experiencing or recovering from mental health difficulties.
2. Provide direct support and advice to Service Users and Carers within the recovery framework, respecting individual choice and control.
3. Assess, identify, and evaluate clients' needs through comprehensive assessments, reviews, and care planning in collaboration with other professionals.
4. Work as part of a multi-disciplinary team, collaborating with stakeholders inside and outside of the Trust.
