Job summary

Central London Community Healthcare NHS Trust is seeking an Occupational Therapist to join the Inpatient Rehabilitation Wards at Potters Bar Community Hospital. You will work within a multi-disciplinary team to assess, plan and deliver patient-centred rehabilitation for a variety of conditions and contribute to discharge planning.

As a Band 5 OT you will manage your own caseload, deliver evidence-based interventions, document professionally, and participate in CPD, audits and service development

Qualifications

• Work collaboratively with colleagues to deliver patient-centred rehabilitation.
• Assess individual needs and plan appropriate occupational therapy interventions.
• Provide occupational therapy assessment and treatment for referred patients, planning, delivering and evaluating interventions for individuals and groups.
• Manage your own caseload autonomously in line with professional standards and clinical guidelines, maintaining accurate professional documentation.
• Contribute actively to the multi-disciplinary team, delivering a comprehensive Occupational Therapy service for residents of West Hertfordshire.
• Build and maintain effective relationships with referring CLCH acute and community services to support appropriate referrals and understanding of the Occupational Therapist and multi-professional team roles.
• Take part in Continuing Professional Development, in-service training, audit and research undertaken by the service.
