Job summary

Responsibilities

• Deliver advanced-level holistic nursing care from history taking to evaluation.
• Assess, diagnose and treat acute and chronic conditions.
• Prescribe medicines and appliances according to patient need.
• Collaborate with the GP team to meet patient needs.
• Refer to specialists or multidisciplinary team when appropriate.
• Contribute to QOF targets and vaccination campaigns.
