Job summary

The Methodist Church in Britain is seeking an Advanced Clinical Practitioner to join the general practice team split between Chaucer Surgery and Camphill Health Centre. The post is part-time (30 hours over four days) with consultations by phone, online, face-to-face and home visits.

The role requires a postgraduate ACP qualification and an Independent Prescriber qualification, or active progression towards the latter.

Qualifications

• Postgraduate ACP qualification or equivalent.
• Independent Prescriber qualification or working towards it.

Responsibilities

• Assess, diagnose and manage acute and chronic presentations across age groups.
• Conduct home visits and clinic consultations (phone, online, face-to-face).
• Prescribe and review medications within scope.
• Order and interpret investigations and diagnostics.
• Refer to MDT, secondary care and external services.
• Support QOF, chronic disease management and preventive care.
• Provide clinical leadership, supervision and support to colleagues.
